Supervitrified Dish .... looking for the number please.

This dish has had a hard life, but as it didn't cost anything I thought I'd better get the shape for the records. We just have to try and work out which shape it is ....
It measures 193mm x 147mm and as the backstamp shows it was made in 1983.
