It sure looks like a sample Forma plate and your post reminded me that I have photos of David Jenkin sample plates from his daughter's exhibition and I will have to find them and post them here when I get a chance.

I don't think that the Joker plate is Crown Lynn Kat as his daughter said it was an early design and possibly it was done when he was at Elam.
